Office Hours
Our office hours are as follows:
Monday: 7 a.m. - 6 p.m.
Tuesday - Thursday: 7 a.m. - 5 p.m.
Friday: 7 a.m. - 4 p.m.
Appointments
To make an appointment, call our office at (904) 641-6110 during regular business hours. Same day appointments are available!
If you are a new patient, please arrive at least 15 minutes early to complete the registration process. Please be sure to bring your insurance card, driver’s license, and any medications with you to your initial visit.
Insurance
We participate with most major insurance plans, including Medicare. Please check with your insurance carrier prior to your visit to ensure that we are still a participating provider with your plan.
Cancellations
If you are not going to be able to make it for your scheduled appoinment we ask for at least 24 hours notice so that we may schedule other patients for that time period.
